This policy explains what Equora Wellness collects when you request an evaluation or become a member, who it is shared with, and how to ask for a copy or a deletion. It describes what this site actually does — nothing more.

Who we are

Equora Wellness operates this website and the Equora membership program. We connect individuals, employers, and residential communities with a vetted network of licensed, insured, and background-checked physical therapists. Clinical care is delivered by independent licensed providers.

What we collect

Booking requests. When you use the booking form we collect your name, phone number, preferred time of day (early morning, late morning, afternoon, or evening), and an optional note you choose to add. We do not ask for a diagnosis, medical history, or insurance details on that form.

Account information. If you create an account we store your email address, a protected password, your phone number, and the records your account shows — appointments, membership status, and billing history.

Payment information. For a membership we keep the card brand, the last four digits, and the expiry month and year so you can recognize and manage the card on the Billing page. We do not store full card numbers.

Usage information. We record general site activity such as which pages are viewed and which buttons are used — for example booking and phone clicks, and when a membership is started.

How we use it

To contact you and confirm an appointment time, to set up and administer your membership, to prepare billing documents, to keep a record of what you asked for, and to understand which parts of the site need improvement. We don't sell your personal information.

Who sees it

Each booking request is saved to our own database and sent as a notification email to the Equora team addresses equorawellness@gmail.com and cbenitez2315@gmail.com, so someone can call or text you back.

The licensed physical therapist you choose sees what you share with them so they can prepare for and deliver your visit. Equora does not deliver clinical care itself — care is provided by independent licensed providers.

We use an email provider to send those notifications and an analytics provider to count site activity. Health details are never sent to analytics.

The optional note box

The booking form includes an optional note. Please use it for scheduling context rather than detailed medical history — your evaluation is the right place to describe symptoms in detail, and that conversation happens directly with your physical therapist.

Insurance and superbills

A superbill can be provided to submit for insurance claims. We are not a network provider for any health plan and we don't bill insurers on your behalf, so we don't collect insurance information through this site.

How long we keep it

Booking requests are kept so we can follow up and confirm the request was handled. Account, appointment, and billing records are kept while your account is open and afterwards as required for our own records and any legal obligation.

How we protect it

The site is served over an encrypted connection, access to records is limited to the people who need them to run the service, and account areas require you to sign in. No method of storage is completely secure, so we keep collection to what the service actually needs.

Your choices

You can ask us for a copy of the information we hold about you, ask us to correct it, or ask us to delete your account and its records. Email equorawellness@gmail.com or call (561) 312-5962 and we'll confirm what we can do.

You can also stop membership billing by requesting cancellation from the Membership page of your account.

Other notes

This site is intended for adults. It isn't directed at children, and we don't knowingly collect information from anyone under 18.

The site links to third-party pages, such as the medical reference sources on our condition guides. They have their own privacy practices, and this policy doesn't cover them.

If we change this policy we'll post the revised version here with a new "Last updated" date.
